Instructional Planning and Strategies
Transition plan are designed by an IEP team to help students with Individualized Education Plans prepare to be college and career ready after high school. In this artifact you will see what a transition plan looks like, how goals are designed, their objectives, and how they are written. The intention of a transition plan is to demonstrate familiarity with a students strengths, weaknesses, and interests in order to create goals on how to make them successful onward. Note just how many individuals are involved with the instruction planning and strategies in one plan. These plans are designed by the students IEP team, and include goals related life after high school that are timely, and attainable. As a special education teacher, I am the lead designer in these plans. Each of theses plans requires a tremendous amount of instructional planning, understanding the clerical aspect of it as well as being creative with a students goals is what makes a successful transition plan.
